What documents you need to provide to apply for a Blue Badge
If you are applying by post send us copies and not originals of the following documents
Proof of identity and Address
You will need to provide:
- proof of identity (such as a birth or adoption certificate, passport, driving licence, marriage/civil partnership certificate, or divorce/dissolution certificate if your name has changed)
- proof of address (such as a recent council tax bill, a government letter, driving licence, or letter from a school if under 16)
- recent passport style digital photo (head and shoulders)
- your national insurance number (if you have one)
- your contact details (phone number, email and postal address)
Proof of eligibility
For each application, you will need to submit relevant evidence to demonstrate that you meet the selected eligibility criteria. This evidence should be dated within the last 18 months to 2 years. (for more details, see Department for Transport guidance)
Proof of benefits
Personal Independence Payment (PIP)
You’ll need to provide the 3-page letter from DWP, showing your entitlement, assessment scores, and mobility scores.
Disability Living Allowance (DLA)
Provide your most recent DWP letter showing your mobility rating and certificate of entitlement.
Armed Forces Compensation Scheme
Submit the most recent letter from the Ministry of Defence, showing your injury details and benefit award.
War Pensioners’ Mobility Scheme
Provide a recent letter from the Ministry of Defence showing receipt of the War Pensioners’ Mobility Supplement.
