How our adult social care team can help people with learning disabilities and their friends and families.

Community Learning Disability Team

The Community Learning Disability Team are a social care team that supports adults with learning disabilities and their families or carers. 

Contact us if you or someone you care for:

  • has a diagnosed learning disability
  • is 18 or over
  • lives in Kingston

What we can help with

We have lots of resources to support people with a learning disability in having independent, safe, and active lives. 

We might be able to help with things like:

  • making choices
  • looking after your health
  • keeping in touch with people you know
  • finding activities to do
  • helping you look after your money
  • finding a job
  • getting care at home
  • living at home safely
  • finding supported living accommodation or a residential home

What happens when you contact us

If you contact us, we will ask you questions about you and how you live. We want to find out what you need help with, and if we are the right team to help you.

Advice and information

We can give you advice about other organisations that can help you or things that you might want to try.

Ongoing support

If we think you need more help and support, we will recommend that you have a care needs assessment with a member of our team. This usually means that we’ll come to your home to talk to you and ask you questions about how you live.

The care needs assessment is a longer conversation to work out if you are eligible for funded care from us. 

Who is eligible for social care

The care needs assessment helps us work out if you are eligible for ongoing funded care from social care.

You may be eligible for some funded social care if the assessment shows that you:

  • have a physical or mental impairment or illness
  • cannot perform some daily tasks because of that condition
  • experience a negative impact on your wellbeing because of these difficulties

What it costs

Speaking to us and getting an assessment is free. If you are eligible for ongoing support, most people have to pay towards the cost of their care. Find out more about paying for social care.
